The Independent Support Service only offer support with Education, Health and Care Plans. If you were to apply for an EHC Plan, as part of the process your child's needs would be assessed by a professional and strategies for supporting that child would be suggested.
The British Dyslexia Association have a small section on dyscalculia

The Dyspraxic Foundation have a wealth of information about dyspraxia

I could not find a specific support website for disgraphia but there are facebook groups, perhaps members there would be able to offer ideas.
An Educational Psychologist will be able to assess whether your child has dyscalculia and/or dysgraphia. It may be that you request an assessment from an occupational therapist or a physiotherapist.
You could also try posting in the Education and Learning section to see whether other parents have had similar experiences
