Need write letter to help Dr give me a letter as evidence don't know where to start

Hi, I rang Dr to ask for a letter for medical evidence for autistic sons review for pip, I don't know where to start, I suffer from anxiety myself and am so stressed at moment I feel like I'm going to pass out, any suggestions on how to do the letter would be great, do I follow the descriptors on his last award and say what his problems are, I really don't know where to start and his appointment is tomorrow morning, can anyone help please

There's a good example of a letter requesting medical evidence on page 16 of this document, can you let me know if you can open this? If you can, the letter on this page might be good to use as a template for your own letter? -
