Hi can anyone help me,I am in the support group I was put in support group Nov 2017,my claim is due to be assessed the end of April 19,I have just received a capability for work assessment form,My health has not improved and my conditions are not expected to improve ,I also received a Daily Living component of Pip,My question is do ,I need to state everything from my original form as well as any new details regarding my health,I am not in a good place with my mental health,and am already getting anxious any help or advice please

Hi.You also need to make sure you send evidence with the form to support your claim because they very rarely contact anyone for this. Be aware also that there's no extension of time given to return the form and it must be returned no later than the date stated in the letter. Good luck.0

Hello poppy thank you I have contacted my GP for an up To date letter ,I have to write down what I want the letter to say and the GP'S sectary will type me one,it costs £11 ,I also have a copy of the letter I got in 2017,I feel a bit of a fruad saying what I want the letter to say,but this is how my practice works,many thanks for this forum it feels good to know I'm not alone,I'm new so hope to be able to help others if I can.As my disabilities change due to deterioration I dont have much new evidence to send,once again thank you

A patient lead letter from your GP where you tell them what to write isn't classed as evidence because you're telling them what to write. Do you have any other evidence you can send? It's free to request digital copies of your medical records from your GP.
