Requested pip assessment notes shocked

Hi
i had my telephone assessment and requested my notes
im really shocked to find no points.
im very confused that the assessor has put thing in like needs help getting in and out of bath but no points?
She has cut and pasted the same paragraph in each box saying I used to drive 2 years ago?? She’s has also said I don’t take daily medications which I do?
I work part time from home and have adapted keyboard and mouse but in the report says I can cook a simple meal because I can use a mouse?
I have RR Ms by the way 
she also said I have yearly scans which is normal but this year I have had 2
I just am so shocked at the report 
do they even look at the medical evidence when they are on the phone to you?
any help would be much appreciated xx

    Hi,

    The report is a recommendation from the AP based on everything you sent and what you said during the assessment. Only the decision maker can award the points.
    The decision maker will look at everything you sent, as well as the assessment report, which becomes part of your evidence. From this, they will then make a decision.
    They do mostly go with the report but it has been known for them to go against it.
    Once the decision's been made if you're not happy you can request the Mandatory Reconsideration (MR).
    I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.

    Thanks Poppy I was so upset reading it!! Everything I told her very personal things (was difficult)
    and she has made no reference to any of it!! 
    Where I need aids she’s ignored it I find it so hard to believe she would do that!!
    I feel like phoning up to complain!! I’m so annoyed

    This is why it's not always a good idea to request the report because it makes anxiety even worse. A lot of people think that during the assessment the AP should write what you say word for word, this isn't what happens, if it was there would be no point in the assessment in the first place.
    They ask you questions, then write the report based on everything you sent and what you said during your assessment.
    You can make a complaint to the assessment providers if that's what you want to do. Though we've had members here in the past that have done this and nothing has come of it.
    Once you've had the decision, if you're not happy post back here and i'm sure someone will advise you further.

    The same happened to me unfortunatley, Assessor basically said I’m capeable of doing every day to day activity which is not the case, I’m expecting the award letter to say No.

    And the Assessor copied and pasted Numerous times that I Have a Driving Licence too, Sounds like we could of had the same Assessor..

    As the dm eventually sees these reports copying and pasting in this way must be condoned, I mean they receive some sort of training to do this, 

    seems odd that the person conducting the assessment isn’t the one who awards points, after receiving a mess of copying and pasting and not passing on what the assessed has said how on earth can the dm have an accurate picture?! Not always possible I suggest, hence the number of MRs.

    That is very true, Multiple times it says ‘He has a Driving Licence’ not that it should matter if I do or not, She said.. ‘He answered all the questions politely’ When in fact I did not answer them as I do not understand complex information, My mother answered them, She basically just contradicts herself on the full report, Hopefully the DM will see this and go against her report, it’s a disgrace.

    The assessor writes a report.
    The DM is supposed to look at the report and all the other documentation that has been sent in.
    Information I post is for England unless otherwise stated. Rules may be different in other parts of UK.

    The assessor in my case mentioned I have a driving licence at every opportunity she could on the Assessment Report, What’s more frustrating is.. They seem nice on the phone.. Then they send a complete lie of a Report back to DWP.

    Not sure I see an issue with copying and pasting. If the same reason has been used to score someone points across multiple descriptors that’s not really a surprise. Why should you have to find 10 different ways to say the same thing? The issue is drawing the wrong inference rather than copying and pasting. The bottom line remains that a HCP can only draw a wrong inference if a PIP 2 has not been fully completed.
    The assessor in my case has definitely got it wrong, I will be making a MR, And taking it further as I feel the assessment was a load of rubbish, It stated on my PIP form my problems, And Also on the Evidence I sent from Autism Diagnoses Services, It seems like what ever she says goes, After a brief 30 minute assessment, Which she lied on that too stating it lasted 45 minutes.

    You applied last year also but were refused back then. Either you don't qualify and some people don't because you can have a disability but not qualify for PIP. Or you're clearly doing something wrong and as Mike has already stated, poorly completed claim packs are often to blame.

    I just don’t get how a ‘Health Care Professional’ can make a opinion of somebody after a 30 minute phone call, I have Autism, Anti Social Personality Disorder and OCD, Barely leave the house, Which I told the assessor, And she thinks I’m lying lol.

    PIP isn't awarded based on a diagnosis. You need to explain the difficultly you have with each descriptor that applies to you. Have you given a couple of real world examples for each descriptor that applies? Fully explaining everything that's happened? You should aim for at least one A4 side of paper per descriptor.

    I know how you feel, I just received my decision back that included the report and it's quite disgusting what they have written, totally ignoring what you have said on the phone. They had written multiple times that I had no specialist input, yet I had sent copies of the letters from " Specialists ", they stated I don't take anxiolytics, when I clearly said on the phone I'm taking Citalopram, which was also written on the PIP form and evidence of repeat prescriptions, plus many more things the failed to include.

    It's basically a joke, the assessors should be ashamed of themselves.

    Also if a person can't walk over 200m, how the hell can an assessor assess that over the phone?

    It’s awful, It basically feels like everything what was said on the phone.. The assessor made the opinion of there’s absolutely nothing wrong with the claimant, Literally ticked every box which states I am able to carry out every day tasks, I’m not sure if this was the same case for you?

    I have not received a Decision yet, But by reading the assessment report it will not be successful.

    Very true..I was only awarded standard mobility after my first assessment because I could drive a car, despite it being painful to do so but they don’t care about that...some of these assessors are not fit for purpose...mine said I could walk fine because I’m swapped my walking stick between hands...which is what my physio told me do if there was a railing on the side I usually hold my stick in...she also said I arrived carrying my handbag, notes and a bottle of water....my bottle of water was in my handbag which my  husband was carrying for me...

    I have a friend who was once a nurse and made the mistake of becoming an assessor...she lasted a matter of weeks saying..in her words “I wasn’t prepared to bend the truth or lie on the assessment reports”

    I had a telephone assessment, She said ‘I’ answered all the questions with no problems, Even though I didn’t I had someone with me to help as I do not understand complex information with my Autism, The only hope I have is that the DM goes against her disgraceful report, It’s frustrating and upsetting knowing a ‘professional’ has not entirely told the truth, Time will tell once I receive the DM’s Decision I guess..

    I did a MR but got nowhere..I couldn’t face an appeal so just accepted what they had awarded..my award was until Feb 2022, later extended to Nov 22 due to Covid...however I submitted a change of circumstances in August and was surprised to hear nothing until an amount appeared in my bank account, decision letter arrived the day after and upgraded to enhanced for both with an ongoing award.  I was gobsmacked as expected another assessment but they obviously did a paper based one which I was more than happy with,
