Daughter moving from DLA to PIP, can anybody advise me on what I should do

hi there, my daughter was getting DLA up until march 2022, I have mental health issues myself and did not re apply since then she has now turned 16 in june of this this year, i am now trying to claim PIP for her and i have phoned several times for the forms and to my dismay they have never arrived the recent forms i asked for was approx 3 weeks ago.
again they still have not arrived, i have also been informed i need an appointee formor letter she has SEN, dyselexia and is in reciept of an EHCP..... I am confused as to what i should do as i am doing the correct things but i am getting nowere fastcan you please give me advice on what to do as she is now starting college very close to home as she is not very street wise nor does she realise the dangers and puts herself in very vunerable situations please is there anybody who can advise me on what i should do

    Are you sure they have the correct address? If you want to become her appointee did you tell them this when you rang to start her claim? If you did then the forms for the PIP can't be sent until the appointeeship has been set up. Once this is done, they will then send the PIP forms to you, in your name. When filling out the form, you do so as if you were her.
    They are closed tomorrow but when they open on Tuesday i'd advise you to ring PIP. 0800 121 4433
