fit note question

Hi all
My problem is i suffer with hEDS ive been suffering for many years i also have slipping rib due to hEDS i have vitamin d deficiency and anemia due to low folic acid i got a fit not from my doctor last month for one month i asked for another fit note and was sent a link in a text to follow i followed the link and requested a fit note dated 23/9/22 to 23/10/22 i received a message 3 days later to collect my new fit note i picked fit note up from doctors and when i got home realised it had duplicate stamped over it. it says date patient seen 26/9/2022 date printed 27th sick for 1 month does that mean he gave me the fitnote from last month the fit note from last month had 28days and was dated 23/8/22 and a different doctors name just not sure about the duplicate part i am currently making a claim for esa and pip as my health coach advised me to i do not want to get in trouble with dwp for giving fake notification for fit note

There should be a start and end date on the fit note, if those are correct then that's fine. You do not need a fit/sick note to claim PIP.

i need fit note for esa they have told me to keep supplying them until assessment i got a questionaire after 3 weeks my fit note has no dates neither of them one says for 28 days the other says 1 month i just dont understant the duplicate in huge writing across fit note0 -
If my fit note was a duplicate from last month would it have last months date on it ive never had a fit note until last month0

There should be at least a start date at the bottom. You can send it and if there's a problem you will be contacted.

There should be a start date so the end date is 28 days or one month later as applicable. If the from date is blank the start date will be the 'I assessed your case' date (or the doctor's signature date).salty1212334 said:..one says for 28 days the other says 1 month1 -

the i assessed u dates are different also doctors signature is different so they must be different thank you both sorry for multiple quotes did not no how to remove them0 -
The assessed dates will obviously be different on each Fit Note. i just did not understand the duplicate writ over it i searched the internet all i could see was if u lost old fit note u can get a duplicate this is what gov uk sayscalcotti said:

Duplicate fit notes can only be issued if the original has been lost. The fit note is classed as advice from your healthcare professional Your employer can decide whether or not to accept it, and your healthcare professional cannot get involved in any disputes between you and your employer
I could phone but will be waiting days for a phone call and my phone is broke i have to keep it plugged in cant do nothing for days and i dont like to bother doctor i feel like im harassing him i saw him 5 years ago if it wasn't for my health worker i would not have bothered phoning doctors i claimed nothing for 3 years as i was in to much pain i get a fever when ever i do anything im taking nerve blockers at the moment im currently really stressed and in a lot of debt from council tax as they thought i was working for the 3 years even though i showed no income to get rent paid i was living off paste on toast for 3 years thats whats caused my anemia cost me 3 pound a week i just dont want to go back to that because a fit note and its not OBVIOUS to someone who has only ever had or seen a single fit note why u reply with critisism is beyond me on a website created to help people0 -

I very often edit my comments if i haven't included all the details or there's a spelling error, it doesn't mean you were criticised.You can't delete your own account. I did reword it because when I reread it I could see that it could be read as a criticism which was not my intention. I have no explanation why it says duplicate but I can’t see that it makes any difference to the use of the Fit Note.
There is however a gap between the two dates.
The Fit Note dated 23rd August for 28 days runs to 19th September. The other Fit Note for one month covers 26th Sept to 25th October.
You can see from the Fit Note that the doctor has the option of entering a period (28 days or one month for example) or entering start and end dates.1 -
