In short nothing will happen.
The 13 weeks that you mention is called the assessment phase, and this will continue until the DWP make a decision on whether ESA can continue or not. Whether ESA continues or not will depend on if you have 'limited capability for work'. Your capability for work will be assessed via a medical examination.
Generally an ESA claim is as follows:
1. Claim ESA
2. Called for a medical assessment (Work Capability Assessment)
3. DWP decide if ESA can continue or not
