Hi does anyone know if I ring UC do you think i could can get an extension on the date the form has to be back as I'm really finding the form very stressful to complete, Thank you.

    They don't extend the time to return the form. If you have a vulnerable marker on your file then they can continue without you returning the form.

    There's a guide online that may help you. The link says ESA but the UC50 form is identical.

    Hi poppy I've just got of the phone to the health assessment people I told them I can't fill the form in its to overwhelming for me to cope with.

    I have 29 illnesses and the form is asking for all the details about each one I can't deal with it this is why I was paper based for pip.

    They have emailed another department to call me to help me with the form I do have a print out with all my illnesses and medications from my Dr's but I don't know if they will except all the information with out me trying to write it all down.

    Hi @anth22, it can certainly be overwhelming to fill out the form yourself. Do you have anybody who can help fill out the form on your behalf at all?

    If not you could ask at your local Citizen's Advice as they may sometimes be able to provide support or point you in the direction of support.

    The health assessment rang me to let me know they have lifted the time frame my form has to be back and I can send it back whenever I'm ready.

    They suggested I get family to help me fill it in as they know me better and how I'm affected by my illnesses, atleast I don't have to rush now.

    That's good @anth22, glad you can relax a little and don't have to rush to fill in your form. Have you got someone who can help you fill it in so it's not quite so overwhelming?

    Hope you can get it all sorted when you're feeling up to it!

    Sounds like you have a vulnerable marker on your file which is why the deadline is lifted. I'm glad to hear this because now It will hopefully be less stressful for you to fill it in and return it.
