I’ve been told to ask for a Pa3 form…
Does anyone know where and how I request mine?
thanks 🙏

PA3 is the assessment report that's written when you have a paper based assessment. PA4 is when you have an assessment. Either way, you ring PIP to ask for a copy of the assessment report.

Thanks Poppy… does the dwp decision making go solely on the Pa3 report? As the assessor sounded as if she hadn’t really looked at the evidence I’d supplied after I’d sent my form back…0

If you had an assessment as i advised it's called PA4 report. A PA3 report is when you didn't have an assessment known as paper based assessment. The report is based on their opinion of everything you sent and what you said during the assessment. The decision maker usually goes with it but it has been known for them to go against it.1
