My Med Records & applying for benefits or reassessment

Hello

I wanted to get my GP summary record to see if I could maybe have it in time for any re assessments but since I got hold of it I’ve noticed most of my diagnoses are missing!

I’m literally shocked as to why

    @bluefox Can I ask, were some of your diagnoses made through the hospital or specialists? I had to get a summary and found that at my GP practice, they had to print it off two different systems, theirs and the one they shared with the hospital.

    Psychiatric specialist yes but she gave them 3 diagnoses and not a single one of them is written in my summary record. Even from a NON-BENEFIT point of view.. if someone was to treat me, they need to see this!

    How frustrating @bluefox :( Do you feel comfortable and able to point this out to your Psychiatric specialist or GP?

    I agree, it is really important your records are as accurate as they can be. If not for benefits then at least for providing you with the treatment and support you need and deserve  :)

    That is frustrating @bluefox Have you gone back and spoken to them about making sure the diagnoses have been logged correctly on their system?

    How would I approach that situation?

    I would call or go into the GP Practice and say you've noticed your summary record doesn't have all your diagnoses, is there any way we can correct this? And they'll hopefully be able to help you from there :)
