    I can't tell you which ones will apply to you (if any) because i have no idea of exactly how your conditions affect you.

    My husband has eupd and severe recurrent depression. He is also at present being assessed for adhd 

    he struggles daily with moods, being angry, erratic and has a tendency to self harm/ break things/ argue and lash out because of this. His depression has him also having suicidal tendencies. He’s on a lot of medication to combat these episodes and feelings but they knock him flat off his feet. 

    So when filling out the form, do I fill it in as though he’s not medicated or that he is, because half the time whilst medicated he sleeps a lot. It’s all very confusing.

    You fill it in exactly how his conditions affect him. If he takes medication then you tell them. If he experiences side effects of the medication then you tell them.

    There's a guide here. It says ESA50 but the UC50 is identical.
