Hi my name is Jamie,can anyone help me with what the best thing to do is about giving your GP CONSENT OR SHARING INFORMATION,is it beneficial for me to tick yes or no,it's notify GP of the outcome of work capability assessment and on page 22,and SHARING INFORMATION ABOUT YOUR HEALTH CONDITION PAGE 18,don't know whether to tick yes or no,hope someone can help,many thanks Jamie.
