Train to plane and other way round?

Hello people,hope u doing well today?
I am a disabled traveller who wants to know how to get from the train platform at Heathrow airport to the airline check-in desk?This is quite a distance,previously I was able to walk/stumble along but my abilities have deteriorated and I need a wheelchair to travel far these days.Will disabled assistance from the train company take me to the check-in desk at the airport?
And how does it work ,when i return? will the airlines assistance go with me after baggage reclaim right through to the train station? ie beyond the basic airport,so I can get a train connection.

Hi @future3000, first of all welcome to the community
The train company will help you from the platform to outside the station. From there the airport usually takes over. For the train you have to contact the relevant train company, for the airport it's done through the airline you are booked with.
