UC50 form - spinal stenosis diagnosis

Hi everyone,
Its been 3 months since my spinal stenosis diagnosis. I had nerve block injections just over a week ago on L2, L3 & L3/4. The pain is returning and I'm finding myself struggling emotionally. The last 8 years have been a big challenge following my Hydrocephalus diagnosis and subsequent brain surgeries which then led to lung complications as the catheter from my VP shunt came loose from my abdomen and then migrated to my lung.
And now this spinal stenosis has again stopped me in my tracks. I've just received the UC50 form to fill in which I think is why I'm now feeling anxious and fed up.
I thought to ask if anyone has had experience with the UC50 form which is pages long. I had just received two new conditional job offers just a week before I had to go in to hospital with the stenosis. I'm struggling with what comes next.
My surgeon has said that he will refer me on to a tertiary unit somewhere if these nerve block injections don't work.. the way its feeling now I'm quite sure the referral is going to happen and I think surgery may be the next step.
