Uc50 original form and notify GP of the work capability assessment.

Hi there everyone,just a quick message,I get very different opinions on sharing information about your health information and whether to tick yes/no,I ticked yes, good idea?and consent to notify your GP of the outcome of the work capability assessment, whether to tick yes/no good idea?I ticked yes?any thoughts on this?All on the UC50 FORM,also my original form UC50 was lost in the post,sent out duplicate UC50 form a duplicate it was stapled, falling apart,and apparently a different colour I received a printed out falling apart black/white form,when the originals I've been told are blue/white, because of the state of the duplicate one sent out,I used a one a friend gave me an original one,no name on it, exactly the same but blue/white,is this Ok?as my friend no longer needed his for anything,can you go into the jobcentre and pick the original one up or are they especially sent from the work capability assessment people?the forms were identical,would it have mattered as I told them the one they sent out was in a mess in a email,But do they have any special numbers or codes to identify specific forms for people as the one the person gave me had a different number on to the duplicate replacement one I sent out,the blue one had 06/23,the duplicate one had 12/20 on it,also can you fill the UC50 form out online.Any help on this I would be extremely grateful,thanks everyone.

    I don't think it matters whether you tick yes or no. Nothing really happens other than they will either inform your GP or they won't. 

    The UC50 doesn't have specific reference number on it for each individual person so it doesn't matter which colour you send because they are all identical anyway. In fact, there's one available online that you can fill in and print yourself. https://www.gov.uk/government/publications/uc50-form-universal-credit-capability-for-work-questionnaire

    For others reading this, you shouldn't fill in the form, unless you've received one in the post because if you haven't been referred for a WCA the health assessment providers will ignore the form.
