Listed medical conditions on UC

when I transferred over to UC from WTC I listed all my medical conditions.
Can I see a copy of this anywhere on my UC online account?
I want to double check a couple I may have forgotten to add 🥴

If you click on "journal" and scroll down you should see everything on there. You don't need to list all of your health conditions when you reported a health condition. Have you provided a fit note?

Thank you. I’m waiting for a GP appointment which is in another 4 weeks to request a fit note.
I looked on my journal but it just says I completed my health section so can’t check what I put down
