pip award review form

I have just received the above form and I'm wondering if I could get help to fill it in, I have episodes of phycosis due to my anxiety and stress, and I can't hold my concentration for long, this form is spiking my stress level already and can't make any headway
it has to be back on the 18th July as well

You can ring PIP to ask for extra time to return the form and they will give an extra 2 weeks.
We can’t physically help with the form but we can point you in the right direction. You need to make sure you treat it as a new claim and include as much relevant information as possible. Please do not just write no change and nothing else.
There’s some information hereYou can also get some expert help and advice from an agency near you. The only problem with this will be getting an appointment in time to return the form. This link will help you find an advice agency near you.

Hi @muso, welcome to the community. Sorry to hear you're having difficulties filling in your PIP form. As Poppy says, perhaps you could apply for an extension so that you can have a bit more time to fill it in. It might make it feel a little less stressful and allow you to take it nice and slowly.
