uc50 form sent to my gp
hi today i looked on my nhs app and have seen that dwp have sent a questionaire to them,my doctor i have seen for years is currently on maternity leave so theres a locom that doesnt even know me ,im on 50 tablets a day including zomorh and oramorph for my serious pain from a lot of complex conditions i self catheterize also ,i have arthritis all over i have a cyst on my spine i have collapsed discs in neck and spine , ihave no carteledge between both shoulder joints , i have no pancreas and in agony due to botched surgery when they lost a stent inside of me . We recently were migrated from income support so now have been putting sick notes in, im on high rate pip and my husband is my full time carer, but im worried that this doctor who doesnt know me will ot give the information correctly,what questions are asked from the doctor worried they will make me go to work and also stop pip this is making me so stressed sorry to rant

about my last post i forgot to say i sent lodas of evidence and all my precriptions ect doctors and consultants letters yet they still contacted my gp i feel they think ive made it all up

It’s mostly a tick box form asking for information. You can see the form here, although it says ESA113 the one for UC is the same.
Sometimes a GP doesn’t even respond. You said you sent evidence to support your claim so please try not to worry too much.

thank you poppy i think im just over thinking but you know what they are like ,my dwp work coach is lovely and has put everything on hold so i dont have to do any tasks but its still worrying
