I think I may be being managed out of work. Am I, and… Do I leave?

I have multiple disabilities but I found a job. I’ve been working for 8months. It’s been good - I have made mistakes but nothing really bad and I’ve never been scolded or anything. I’ve felt like the job isn’t a great fit for me but I haven’t really struggled if you get me.
A bit ago my manager pulled me aside and talked about a mistake I had made where I hadn’t input important data onto the database. The thing is I am adamant I have done it. She pulled loads of papers from the bin and I am in the process of re-checking them, so far every one of them is fine. I genuinely feel gaslit lol. I did make this mistake once but realised immediately.
A few weeks ago prior to this big mistake someone mentioned adding a new team member to do x, x, x, I was sat there mentally going… but that’s my role, but that’s my role?
My manager also told me to go on the sick, which I thought was strange because I felt mentally well, it’s not like I’m going in crying.
I feel like they want me out for someone new. I second guess myself a lot now at work and actually feel dreadful about it. I want to leave as I don’t feel capable of working a job or coping in the work environment, but I am also scared of relying on UC/applying for LCWRA.
Just venting really
