How to fill in UC50 as an appointee

How do I fill in the form as an appointee? Do I use the person's name or do I write it as if it was them speaking ?

Hi @Ikonik I'm so sorry you've not had a response to this yet. Have you managed to make progress since? You may likely have already completed this by now, but if not I'd suggest completing it with the person you are appointee for, answering on their behalf. You could always pop a note to say it's the appointee completing the form. They should have this on record.
