PIP review - is there a difference in the form? Don't know how to answer it, any advice?

I have been on this forum quite a lot lately, due to migration esa to uc ( still ongoing) and now I've recieved my PIP 1043 review form. The last PIP review form I completed was 2022, has the form changed since? As I'm absolutely baffled by the questions & how to answer them. Things not taken into consideration, which apply to me. I have been on full enhanced PIP in two 4 yearly awards. But reading this form today, I may as well through it in the bin.

I know I'm exhausted with illness & worn out with the ongoing migration, but this PIP review form, I honestly dont know how to answer it. A yes or no question if u answer yes its then about 3/4 other questions & things not taken into consideration.

This form as to be back by the 19th Aug & I need help to fill it out & also the copy of my previous review will take up to 10 working days, so I'll have to ring for an extention as I've lost a week waiting for it to arrive. Both my last reviews were paper based.

I really dont know if I'm being dumb with brain fog with everything else going on or the form as changed? I just cant seem to take anything in, its like my brains gone to sleep.

Can anyone advise if they've noticed a difference?

    Hi, my suggestion is to answer the questions in full detail (even using additional paper) the main thing is to explain how your health conditions affect you daily. For example, "when I try to make a cup of tea I unable too because of the trembling and shooting pain in my hands and fingers this a a side affect of then (whatever health condition) you have.

    The main point is they want to understand how your health conditions affect you daily and send as many copies of your diagnosis that is GP letters, Consultant letters, Result of scans, xrays, prescription list.

    Also maybe ask citizen advice or a community based welfare advisor that can help you complete the form. Best of Luck

    Its not about answering, the form is just so different. I have attached the Preparing food & cooking as an example from todays form.

    We do not include carrying items, etc now written on the new PIP1043 review, these comments werent there on the old review form I completed in 2022.

    I have to include it I cant carry or bend as thats part of my illnesses. So I simply dont know what to write.

    I feel like saying to each descriptor in each box, please see covering letter & type up everything & just enclose it in the form.

    The Benefits & Works site says To copy & paste from original PIP form, as my conds havent changed & will never get better only decline. All this was written on my 2 previous reviews & awarded paper based & stayed on enhanced for both. But the reviews I had hand written, this time I dont think I can explain myself the way form is designed with the " we can consider things like" & " We do not consider things like".

    I am honestly so confused, maybe its worse as I'm doing both migration & now PIP review & my brains mushed, fogged. I feel blank, I cant even write my own name & meds on it, like I've frozen!

    I have a support worker who will help, she as helped me with the migration. But she only works short hours.

    My only changes are meds increased, additional meds, colitis flare ups which which I had never mentioned before, as they didnt really affect me, now its daily & back on meds for that. Plus undergoing uterine/ovarian cancer tests, I had my last MRI with contrast last Thursday, thats 5 scans in 2 weeks plus bloods & waiting for MDT to decide results. Even my hair is falling out now.

    This PIP 1043 says not to inc GP letters, but GP wants to get involved as I was discharged from Spine clinic, 3 vertabra fractures, lower lumber collapse of my spine & swelling around discs ( sorry I cant remember the medical terms off the top of my head) as no more can be done, pain relief & GP to monitor & my SSHL ( sudden sensorneurnal hearing loss) I went deaf over night, which as caused a multitude of side affects as my brain cant cope with the sudden loss. Side affects, vertigo, dizzy, poor balance, no coordination etc the list goes on. Again discharged to my GP as no cure for this & hearing aids dont work, they make my side effects worse. I lip read & have on 40% vocal clarity in my left ear, the rest of my hearing as gone & been replaced with side effects, a right side periphial dysfunction. Severe anxiety, depression all of which have become worse over the last 3 months. I spoke to the mental health nurse in my surgery & I waiting on one to one counselling, but theres a 6 month wait list.

    Everythings a mess. My GP & mental health nurse says I am suffering with PTSD since my husbands stage 4 cancer diagnosis in March, somehow its taken me back to my sisters sudden death from cancer. So I am not functioning normally.

    I'm just so tired & trying my hardest to cope when all I want to do stay in bed & cry, I dont see husband anymore I see my sister & its hell & all these forms & changes, treatments side affects.

    I'm sorry for the long post, but today/tonight I feel really bad, really struggling…sorry & thanks for your reply.

    Hi, firstly sending you the biggest virtual hug. ive been where you are and I am new here and only came in looking for advice on this very form , as i have just received mine with no warning and already had to ask for an extension as they wanted it back on the 10th august with 10days allowed for it to get there , i would need a time machine ! Yes the form is different but I have decided to fill each box with please see separate sheet and then typed the name and national insurance at the top and then the question number i am responding to . I cant write for long without pain so i have speak to type software . I just assumed the review form is basically a new claim form in the updated format ( so new tricks ).

    Well I have given up. Its bad enough the migration from esa to uc, then a PIP review. Now I've just had an ESA letter saying cont based have to see a work coach at job centre to explain how my illness affects me. I'm in the support group IR ESA. No idea whats going & feel sick & feel faint
