WCA withdrawn after waiting 12 months

Hello, I was hoping to get some advice.
I'm currently on UC and I started submitting fit notes from May 2024, recieved the WCA form in July, completed and sent back in August, before due date. I then had the assessment on October 8th 2024 via video call. I've waited patiently for an update, around the 6 month mark I queried in my Journal and was told to contact the medical services, when I've contacted medical I've been told its with UC. I kept being told there is no timeframe so I kept waiting and now it has been exactly 12 months since my assessment. I queried again, rang UC to be told no update and to speak to medical and vice versa. I explained all of this on my journal and have now finally been told that according to the medical service my WCA was withdrawn in August 2024 because I didn't send the form back.
I'm absolutely baffled. I had an assessment after they recieved my form, the assessor was clearly referring to my form as I was being assessed. My doctors recieved a medical request form and sent that back also, which would of been requested after recieving my form!
I have explained this via Journal and am awaiting a reply but I don't know what to expect to happen next or what my next steps are regarding this. After waiting all this time I feel so defeated.
Any advice would be appreciated, or if this has happened to anyone else and what the outcome was. Thank you

I would lodge official complaints with both UC & the health assessment agency.

The waiting time alone for the outcome is unacceptable.
