Once ESA have confirmed your previous award with UC, you'll have LCWRA automatically on your claim. This is usually done in time for the first payment statement, but sometimes there is a longer delay due to the two departments not being able to talk to each other efficiently, and the numbers migrating now all letters have been sent out.
Everyone has to agree commitments for UC, for someone with LCWRA it will be to keep them informed of any changes.
What you see will change as UC gathers bits of information about you on the system, as of now it knows little other than you've applied and have health problems.
Every claim to UC is treated as a new claim, so the language and terminology used don't account for those migrating from legacy benefits.

Just put down your best estimate, as I said the form treats everyone as a new claim, doesn't account for those migrating who will be given LCWRA automatically, so it really doesn't matter if you get a date wrong.
