Copy PIP template/form please. Assistance Dog/Mobility

Hi, just completing appeal re PIP.
Challenging Mobility 11f about needing an assistance dog for familiar journeys.
I have the guidance ie that 11f covers the issue but need a copy or detail of the actual page that the original question is on - im sure it didnt allow me to tick a box to say about assistance dog though i did add it in the additional info bit.
So just a copy of the actual question/options on the original pip application form

Hi @Rubydoo, I've moved your post to our Ask A Benefits Advisor category to see if they can advise, however I'm unsure whether they will be able to provide a copy of the actual application form.
However, this Citizens Advice guidance gives good information and pointers on how best to respond to individual sections of the applications, so it may be worth taking a look at. This section in particular mentions the use of guide dogs. Hope this helps and good luck with your appeal! -

It does seem to mention guide dogs, so if you haven't ticked this, but do actually use a guide dog to navigate then you'll just need to explain this.
