How do I apply for pip

ive been diagnosed with acute chronic pain in my back and legs and I’ve also have been diagnosed with severe depression and anxiety. I’ve been advised to apply for PIP but I’m not really sure on how to go about it. I’m trying to apply because the pain is progressively getting worse now and I’ve been told that it’s highly unlikely to get any better
