Newly disabled - how can I get help?

I have recently been diagnosed with Rheumatoid Arthritis, and Osteo Arthritis.. It took months to get diagnosed as I was sent to every hospital around Oxford.. then Rheumatologist was shocked to see how severe it had spread through my body.So now I’m off sick from work as it’s all effected my health, white blood cells, Rheumatoid levels shot through the roof..
Im now classed a Disabled.. so iv got OIP forms here.. And wondering how I get bus pass and blue badge, as I can’t walk far, so a friend gives me lifts when able to.. Any advice please.. as I’m completely shocked this has happened to me.. many thanks x

    Yes I too find it a maze 
    I become wrong since last yr,
    So I don't travel or use public transport, 
    I need home visit for everything.

    Sorry spelling hard .
    I inform only yesterday photo ID bus pass yet I not allowed on buse rules medical loan indoor outdoor medical loan specialist wheelchair.

    If ever I get offer after my care act assessment process, ot assessment too.
    I shall hope I get too key worker to help things what is or could be offered,

    All I able offer my experience if any help,
