Hi there,
I've just received an ESA3 (IBR) 06/18 form to complete and wondered if anybody had a list of all the questions anywhere? It is because I need to write the answers out on my computer for somebody else to then copy into the form on my behalf. Thanks in advance…

Hi Steve,Thank you for the offer but all I'm really needing at this stage is a list of questions, if they exist in digital format online somewhere? If so I can consider them and dictate my answers via speech software to then print them out for my form filling advocate… :-)Thanks,Mark
