Holiday with disabled child

My partner and I have decided to go away for a fun weekend with the kids in tow. We are planning on driving to Bath this summer. 

We are planning on booking a room at a hotel or BnB. How do I politely find out about disability friendly activities and accommodation? If you can recommend anything I will definitely be grateful. Where do I find information on where to eat and what to do in the city as well. Thanks in advance.

Thanks for your suggestions. We are going to Bath tomorrow morning for five days. I have packed a suitcase of things for our trip. One more question can you recommend any museums? Preferably ones that are generally tolerant of children.
