Filling out pip need help

I have overwhelming evidence for my pip review form, but difficulty on one quesestion able to make a journey. On my original pip claim I could go to the Dr's only with 2 trusted friends who care for me and only in back of car of trusted friend. No other journeys but since 2018 I have had all medical slam and support services in my home as it is to overwhelming to leave front door. How do I word this I have cpn letter Dr's letter and carers and support workers letter stating I am house bound and have home apts. Due to significant reasons and I am unable to leave my flat. Its just on the frm how to write it I'm stuck

These are my top tips for everyone filling out a PIP form:
1-Answer all the questions honestly. Use the spare boxes to give them more information on your impairments too. Remember they do not know, it is up to you to tell them what the nature of your condition is and how you are affected by it. Consider help needed as well. Give examples to support your application form.
2- Do not stress. Don’t be afraid to ask for help if needed. Do you have a social worker or not? Get him or her to check you have answered all the questions properly. Take breaks from time to time to rest your hands and brain.
3- What do you require assistance with? Before you start, my advice is to spend some time thinking about what you find hard and make a list that you can refer to as you fill out the form. Read the descriptors carefully. Take a look at this info booklet. It contains a list of activities plus tips for claimants.

4-Gather as much evidence as needed. Make photocopies of letters from specialists and therapists and mail them off with the claim form. I suggest you also use the diary to help you (you don’t have to send that off however). Think of it like a dreaded exam paper that may help.

Tell them about your good and bad days. Explain why you struggle to trust others as well. Briefly mention the crimes that happened to you. Be honest as much as possible. Describe other debilitating symptoms like panic attacks, confusion or anxiety. Say you are experiencing flashbacks from being sexually abused. Remember that this question is only partially about learning difficulties, it also is about your mental health.
