Have to fill in a Work Capability Assessment Form (UC50) - Any advice?

Hi,
I have finally received a Work Capability Assessment to fill in from the job centre, is there any tips anyone has on how to fill it in.
I'm going to fill in some of it and then pass it onto my nurse to help fill it in so any advice would be much appreciated.
