Request for EHC needs assessment. How much do I need to put into request letter?

How much stuff do I have to put into the request letter? The threshold should be relatively low so ideally I would like to avoid putting too much into the request letter. Are there any sample requests for different conditions eg adhd/ASD?

Hi,Are you getting some help and support with this because it's not a straight forward or easy process, especially if you don't know what you're doing. There's some information here.

Hi @Cmoroz
Thanks for your great question. In theory, it is not about the amount of information you provide, but rather the types of information you provide.
For an EHCP, you need to evidence that the individual you are applying for needs more or different kinds of support than that ordinally provided within their educational setting. I hope this helps.
The link @poppy123456 provided for IPSEA is particularly useful. Another resource you might find useful is https://www.guidedogs.org.uk/getting-support/information-and-advice/education-support/your-childs-education-health-and-care-plan/drafting-your-childs-education-health-and-care-plan/.
I appreciate the individual you are applying for may not have visual difficulties, but Guide Dogs have produced an amazing information section that can apply to all SEND.
If we can do anything to support you further, please don't hesitate to reach out to us0
