LWCRA Awarded....back pay query

Hi I was wondering if anyone could help me or confirm if i worked out my backpay correct? 
Notified DWP of ill health on 01/07/21. My assessment period is from 6th of the month to 7th.
So will I be awarded from October/November 21, and also be 5 payments of backpay due?

Notified in June-July AP.
Three month waiting period is the three APs July to October.
LCWRA payable for October to November (the November payment). Four months owed.
