Hi does anyone else have anxiety filling in forms. I am at the stage of renewal my Son pip he has autism and mental health issue. I find it hard to write the issue he is facing and the more i answer a question the more i wonder if I am makjng sense. Has anyone else experience this if so any advice.

As @Biblioklept has said, I think this is something many people experience, so you're certainly not alone in feeling that way.
Some people find it helpful to keep a diary of what happens when they try doing activities related to the descriptors (pipinfo.net). They then have some recent examples to include. You can read about that on the Citizen's Advice website.
