hi I need some help filling out this form I have been sent it from uc I've read lots of different ppl accounts on this form. is there anyone that I could go to for support on filling it out. thnx Sarah

I don't personally help anyone other than to advise on the forum because i simply don't have time in which to do that. I know scope don't help with that. All i can do is advise you.When filling out the form you should put as much information as possible about how your conditions affect you. Include a couple of real world examples for each descriptor that applies. Include detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.It will help to have a look at the descriptors to see which one (if any) you think applies to you.
