Health Professional report without assessment

Received a text last Friday from PIP stating they had received the Health Professional's report following my request back in May for a review after a back to back NSTEMI and STEMI heart attack in February, followed by a Stroke in April. Not been requested to have an assessment either face to face or by phone.
The review request was completed on the standard form, not the terminal illness form. In all the years I've been claiming DLA/PIP, the reviews have always had an assessment. Is this normal?

Paper based assessments are rare but definitely possible.

I had a paper based one.She called to confirm 2 things but the rest she did. I am awaiting for my report to be posted.1

Phoned PIP earlier today to request the report and was informed the decision was made yesterday.
Now on enhanced daily living and retained enhanced mobility, 3 year award.1 -
