PIP assessor blatantly lied and I got 0 points for mandatory reconsideration

Hi everyone. I realise many disagree with what is written on their PIP assessment but having asked to see mine after once again receiving 0 points she has lied. 
She has said that the assessment was done via video call. She has described me as well dressed, clean and tidy, my hair well kept, shaved, I didn't shuffle in my seat, kept eye contact and appeared to have no issues with anxiety. 
The problem is at the assessment her laptop didn't work so the assessment was done on the phone. At no time has this woman seen me!!! yet she has actually described what I looked like. How can she get away with this. Surely this is a serious lie. Who can I complain to (yes I have proof on my phone that it was a 46 minute telephone conversation at the exact time she claims we were on video. Surely they can't do this to people

    You can complain to the assessment provider.

    I can see why that'd be really frustrating for you to read. As calcotti has said, you could look into complaining to the assessment provider. Would you feel comfortable doing that?

    I am sorry to say this happens a lot, we had the same issue, I say we as my partners carer and support, so I was there and witnessed it happen. We complained "We found some errors, but overall, it was accurate" went to next stage, same response, escalated again, another review found the same. in the end it went to the independent case Examiner, turns out she was in the same office as the last review, she told us they had conferred over the case, while admitting there were "errors" they were happy with the assessment, only avenue left was our MP and case review.

    My partner had a brain injury during an operation, she now has issues, "Fit and able to walk" not true

    Put down as a Full range of motion, she had a fall recently and was unable to even sit on the couch without help, failed the range of movements, did what she could without causing any pain.

    Almost 18 months later we get a call from the clerk of the court, they informed us we need not attend, the doctor had reviewed her consultant's report, other medical evidence and gave a 10-year award. They asked why the case was sent as the evidence alone was sufficient to decide the case, our assessment was just a nurse, the doctor knew what was wrong.

    Your best option is to just live with it unless you can face the long struggle, all our paperwork when appealing was at the Assessment Services head office, there in no Independant Case Examiner, its a fraud to make you think there is an appeal process.
