GP sent a questionnaire or letter still waiting assessment

My GP has been sent a letter to fill in he sent it back but i have not had my assessment do they ever go off what u wright plus evidence from GP or is that only pip i filled the form in with no help

Most people have assessments which can either be by telephone, face to face or video call. Paper based asssessments are rare but possible. If an assessment is needed you will be contacted.
