Struggling writing a letter of support.

My son is on the pathway for adhd diagnosis. He has a few other things going on and already been identified as having difficulties requiring support.
I am applying for dla for him as he has extra needs that go above a child's of the same age.
I want to write a supporting letter as his parent-carer but unsure as what it should look like. The form will have have a clear description of what we have to do daily and nightly for him and how long each task takes. Seems madness to repeat all that in a letter.
Do I just write out what I do to support him and how much supervision he needs? He has emotional difficulties too along with behavioural difficulties and hyperactivity etc.
I am going to include a daily diary of his behavioural difficulties too.
