How do I get a mandatory recommendation form for PIP?

How do you get a mandatory recommendation form for PIP.
I need to be able to draw the DWP to this historical upper tribunal case has my pip as been rejected.
On a different note do you know where I can get decorating and furniture grants etc. I'm on LCWRA?

You don't specifically need to use this as I have said above, I just thought it'd be useful to link with you asking for a copy of a form to complete. A normal piece of paper will do, you must make the request clear with the heading 'Mandatory Reconsideration' joined by your NI number and name on it (do the same for additional sheets of paper too).0
