Hello Leroy1962In my opinion you have 2 options.Option 1 - Telephone the PIP help line and request a new form and an extension to your deadline because you made a mess of your origional form.Option 2 - Do you have a Computer and Printer? If yes then you can type out in detail your personal anacdotal evidence and then print them out, write on each sheet a page number also include your name and national insurance number on every additional sheet you include and then add them with your form and reference the additional sheets on your PIP2 Questionnaire form in each of the activities that apply to you.If you do not have a computer and printer then you can do the same as I have described above but use lined paper and a pen. Here is some advice on how to fill in the PIP2 questionnaire form https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/help-with-your-claim/fill-in-form-pip/Hope that helpsNasturtium

Hello @Leroy1962
I see our members have given advice on requesting or printing a new form. I just wanted to comment and check in with you. Have you got support around you? You can always call Scope's Helpline too if you feel you need support

Yes thank you I’ve got support, just not on the day I was filling form out, due to drinking & anxiety I I just wrote things that weren’t very informative to my condition0

I'm glad you have support around you!
I hope we can be here for you too in the community
Filling out PIP forms can be very anxiety inducing! I see Nasturtium shared the Citizen's Advice link on filling out the form which many members have found very helpful!0
