Has anyone filled out a UC50 form

There's a guide here https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/fill-in-form-esa/ it says ESA50 but the UC50 form is identical.

There's also an online form here if you find it easier to type rather the write. https://assets.publishing.service.gov.uk/media/609bd924e90e07356e2bbbbd/uc50-interactive.pdf you will need to make sure you download it and print it off.

Please do not return that form until you receive the UC50 form in the post. This is because returning it before you've been referred for the WCA will mean they will not accept the form.
