Help, so confused about Permitted Work Form

Please please can someone help me? I'm going round and round in what to do? I am on ESA and PIP and started an Etsy shop in 2020 (it didn't really take off until now) and didn't realise that I had to let the DWP know.  I never work more than 8 hours a week and I have only earned a max of £111 once (i.e. that was my biggest week in terms on sales).
I rang the DWP and they didn't seem to bothered as I don't earn enough or do enough hours but send me out a permitted work form.  I am sooo worried though.  Do I put on there that I stated my shop in 2020 or will I get into trouble because I didn't declare sooner? 
Also the Permitted Work Form is really difficult to fill in because it asks things like
Is my work self employed? - Well is it? I don't have a business officially. 
How much will I earn after tax? - It varies constantly and I don't pay tax
How many hours will I work? - It varies and I never know
When will I get my first payment? - I did, back in 2020????
Supported Permitted Work - I don't have a support worker

I don't know who to turn to to actually get advice on whether I will have my benefits taken away and also to guide me through filling out the form.  I am so worried about what to do.  I range Citizens Advice and they just said to fill out the form as best I can. The DWP ESA guy that sent me the form basically didn't know what Etsy was!

    Ignore the supported permitted work. You are self employed. The tax question is likely irrelevant because the tax free allowance is £12,570/year and it seem very unlikely that your earnings are more than this.

    Ideally, you should have reported to ESA that you were working when you started in 2020, so this is what you should put down on the form. For the hours then providing your earnings haven't exceeded the permitted work amounts per week since 2020 then you will have been working less than 16 hours/week.

    Welcome to the community :) I see Poppy has been able to advise. I know it's easier said than done, but try not to stress and please keep us updated. I wanted to comment to say I am glad to hear your Etsy store is taking off :) I hope it continues to!
