Dose any one have any examples on uc50 with depression anxiety and PTSD

They don't need to know your life story. You should also give a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you. Giving detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.There's also another general guide here. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/fill-in-form-esa/ it says ESA50 but the UC50 form is identical.There's also an online form that you can fill in, download and print off, if you find writing difficult. https://www.gov.uk/government/publications/uc50-form-universal-credit-capability-for-work-questionnaire make sure that the font isn't too small to read though.Don't forget to include your name and NI number on everything you send. Also take copies of everything before you return them.
