I really need help to fill out work capability assessment form. I suffer from learning difficulties and server epilepsy I also am really worried I will fill out form wrong. If anyone can help would be extremely appreciated.

Before filling out the form you should have a look to see if any of the descriptors apply to you so then you know what to concentrate on when filling out the form. You can see them for LCWRA here. https://www.benefitsandwork.co.uk/universal-credit-uc/uc-faq/limited-capability-for-work-related-activity If it's ESA you're claiming then Support Group has the same descriptors.

There's also a guide here that may help you. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/fill-in-form-esa/ If it's UC that you're claiming then this guide applies to the UC50 form as well because both forms are identical.

You should also give a couple of real world examples of exactly what happened the last time you attempted each activity that applies to you.

If you need to use extra sheets of paper then don't forget to add your name and NI number to everything you send.
