Scoring 12 points for following and planning a journey doesn't automatically qualify you for a blue badge. You can apply and send evidence to support your claim but you may need an assessment before a decision is made.

You can apply for a blue badge online from your local council. If you have any letters about your condition just attach them to the application. I asked the my therapist if she would write me a letter and she obliged. I attached the letter to my application and got awarded the badge. Also I did not have to go through any interview process.
Hope this helps. I have just noticed poppy123456 as basically said the same thing.
