I'm no expert on this, but I believe this might be something you need to do at a post office that deals with vehicle tax. Here's a document I found from a quick search. I changed my car but lady in post office said I needed to call them direct and the place I bought it from said the same. I’d claimed it on my previous car last year.

As you've changed your car the only way to do this is at your local Post office. You will also need a new certificate of entitlement because you can't use your previous one. To get this you will need to ring the benefit department that pays your disability benefit, such as PIP etc.
The reason it can't be done online is because your V5C will need to be changed to disabled class.
