Report a change of health and fit note

Hello everyone,
Our UC is currently being processed after moving from WTC. We get the result on the 28th Feb. I currently work part time as does my wife but I am due to have an operation on the 16th March and will be off work for several months. I understand that I have to report the change on my journal but how do I send them a copy of the fit note, do I just take a photo and send it in or do I have to scan it (pdf) and send that.

Usually they will ask you to provide the details on the fit note. If they want to see a copy of it they will ask you.0
