Hi, my doctor has received a form from UC what should I expect? Why has my doctor been sent this, I don’t really understand. Thanks

It is for my UC50 application

Although they don't don't do it ss often as they should, it usually means they are contacting your GP to get their view on your illness and it's impact.
It's actually a good thing and "may" lead to a paper based result.

It will be the UC113 form, which is a simple tick box form for them to fill in and return. You can see that form here, although it says ESA113, it's the same form.
