Migrated to UC from CTC confused.

So we migrated as per migration notice. What a palaver. Went for the F T F to prove we existed π, found it all quite distressing actually. Got a parking ticket in the process, not impressed. Hubby gets carers so they said he's ok. They then told me I would need to look for work ect, but I'm not great physically or mentally at the moment so they said I could have my meetings by phone and to get and send in a sick note. I did so and now have a form to fill in , a UC 50, has to be back by 21st. Is there info or a guide on here to help with this particular form.

The UC50 form is for the work capability assessment. There's a guide here for that. It says ESA50 but the UC50 form is identical.
If you prefer to fill it in online and you have a printer to print it off there's a form here.
You should also have a look at the descriptors for LCW and LCWRA before filling it in.

Just one needs to apply for this or substantial risk if no descriptors apply.
Make sure to fill in the form with as much relevant information as possible. You need to make sure the form is returned by the deadline in the letter.
