Form UC50

hi me & my husband have just migrated from income support where my husband had the disability element included to UC and with husband being my carer he hasn’t any need to look for work but even though I am registered disabled and receive PIP for day to day and mobility I have been assigned a job coach and have to complete UC50 but one question asks if any other person such as carer, relative, family or any other healthcare professional who knows about my disability. Would I be right in thinking I can put my husband’s name as he knows most about how I am affected

You can put your husband and any other medical professional that maybe involved in your care. They very rarely contact anyone anyway so please don't expect them to do that.
